HOW TO ORDER
What is involved
in ordering a home sleep study?
Step 1
A home sleep study requires a physician's order.
This can be obtained by visiting your physician and describing
your symptoms or by taking along a copy of the sleep quiz results,
which indicate a high probability of sleep apnea. Then, for
your convenience you can print out a standard physician's order
form if required, and after the physician completes and signs
the form, fax it to us at 800-278-0706 with your order.
Step 2
Once we receive your order/payment along with
the physician's order (referral for sleep study) we will then
contact you by phone to set up an appointment. After the appointment
is set we will confirm via email. We will then ship you overnight,
the sleep testing equipment.
Step 3
The package will include: recording device
(ApneaLink), Quick Set-up Guide, nasal cannula, abdomen belt,
and oximeter sensor clip. Included also is a pre-labeled shipping
container to use when returning our equipment. Additionally,
as a courtesy, we will provide you with an after hour customer
service number to use in the event you have any questions.
Step 4
After you complete the sleep study simply pack
up all the items provided into the pre-labeled container and
contact the shipping carrier to have the package returned.
Step 5
Upon receipt of the ApneaLink device a polysomnographer
scores your results and the physician reads and evaluates your
findings and recommends (if necessary) the appropriate equipment
to treat obstructive sleep apnea. Only you and your referring
physician receive the findings of your sleep study, the findings
are totally confidential.
