• Complete a Sleep Questionnaire and bring it to your primary care physician.
  • If your physician suspects that you may have sleep apnea, he or she will write you a prescription for a home sleep test.
  • Once your physician has completed and faxed the prescription form, (a copy can be obtained by clicking the prescription form link above) a representative from My Sleep Apnea will contact you to obtain your insurance and/or payment information and to schedule your home sleep test.
  • After a payment method has been established, we will pre-program and pre-connect the ApneaLink for use prior to your receiving it. It’s recommended that you take the test the same day that you receive the ApneaLink.
