- Complete a Sleep Questionnaire and bring it to your primary care physician.
- If your physician suspects that you may have sleep apnea, he or she will write you a prescription for a home sleep test.
- Once your physician has completed and faxed the prescription form, (a copy can be obtained by clicking the prescription form link above) a representative from My Sleep Apnea will contact you to obtain your insurance and/or payment information and to schedule your home sleep test.
- After a payment method has been established, we will pre-program and pre-connect the ApneaLink for use prior to your receiving it. It’s recommended that you take the test the same day that you receive the ApneaLink.
- There are three choices that you have regarding your actual setup.
- We will schedule an appointment for one of our certified sleep specialists to visit you at your home and instruct you on how to take the test.
- You can make an appointment to come to our office and pick up the testing device and receive instructions at that time.
- We can mail the equipment to your home along with a box for you to use to return the device (a nominal shipping charges will apply).
- To ensure the fastest and safest delivery we will ship the device via UPS prepaid.
- Please retain the UPSP box and all its contents, as this is how you will be returning the package back to My Sleep Apnea.
- The following should be within the UPS box:
- USPS prepaid return box
- Blue carrying pouch
- ApneaLink unit
- Black Velcro belt
- ApneaLink cannula – (only 1 is provided)
- Extension cable
- Finger sensor
- Sensor clip
- 2 AA batteries
- My Sleep Apnea written instructions
- My Sleep Apnea, instructional DVD
- My Sleep Apnea supplemental instructions & troubleshooting guide
- Work order
- AOB paperwork
- The following morning, make certain that you have packed the following items into the prepaid USPS box that was included with the ApneaLink:
- Blue carrying pouch
- ApneaLink unit
- Black Velcro belt
- ApneaLink cannula – (only 1 is provided)
- Extension cable
- Finger sensor
- Sensor clip
- 2 AA batteries
- My Sleep Apnea written instructions
- My Sleep Apnea, instructional DVD
- My Sleep Apnea supplemental instructions & troubleshooting guide
- Work order
- AOB/Medical Release paperwork
- Do not ever take out the batteries from the ApneaLink, as all information will be erased.
- Do not forget to turn ApneaLink off in the morning so as not to run the batteries dead.
- After all required contents have been packed into the USPS box, seal the box and either take to your local post office or call USPS for a scheduled pick up at no cost to you.
- Once we receive the returned package, we will immediately process the data and have it assessed by a physician. Your physician will be faxed the results as soon as we have them. It will be your physician who you can contact for the results.
