Scheduling a Sleep Test is very easy. If have a prescription for a home sleep test simply give us a call at 239-851-9797. Our customer service representative will assist you in selecting the best means of getting you the sleep test equipment.

Certain information will be required prior to the dispensing of the sleep equipment. The test does require a physician’s prescription, so if you don’t have a prescription you will have to schedule an appointment with your primary care physician and explain your sleep issues. An acceptable sleep questionnaire and a physician’s prescription form are available for you to print and bring along to your doctor’s visit.

Once your physician has made the diagnosis and completed the prescription form, it can be faxed directly to us at 239-332-2356. When we receive the completed prescription, we will immediately contact you to schedule your sleep test.
